Hours - 1st Shift - 8:00am-5:00pm - Remote Position (Rotating Weeknds, 24 hour On Call Rotation)
General Description of the Job Class:
Inde pendently performs specialized or advanced healthinformation activities necessary to organize, maintain, and use electronicpatient health record s. Activitiesrequired include research, analysis and compil ation of findings into summarizedreports that will be shared within and external to the Health InformationManagement Department. P ositions at thislevel have high customer service, strong analytic and problem solving skills,require interpretation and explanation of policy an dexternal requirementsrelated to, privacy, release of information, posit ive patient identity, andother department and organization-wide function s. Specific job responsibilitiesarebased on the service unit within Hea lth Information Management assigned. Expertc omputer skills to navigate and query multiple electronic record systems.
Patient IdentityManagement: regular interactionsrequired with pati ents, attorneys, insurance companies, providers of healthcareand governm ental agencies. Detailed workingknowledge of and abilityto apply State and Federal laws regarding positivepatient identification, identity theft, vital records regulations, JointCommission requirements, and CMS guidelines and rules.
Duties and Job Responsibilities:
- Track, monitor and validate patient overlays that occurduring patientre gistration and scheduling activities. Analyze and trend datafor feedback to appropriate department. Notifies compliance andaffected department s of incorrect documentation location. 25%
- R esearches through comparison of demographic andclinical information and resolves patient identification problems referred byexternal departments or patients for fraudulent identity use, such as identitytheft. 15%
- Performs concurrent and retrospect ive contactmoves. Analyzes thecontent of healthrecorddocum ent, coordinates with responsible providers, clinicians, andancillary de partment participating in the patient's care for record correctionrequir ements to resolution through Chart Correction Workspace. 15%
- Leadsclini cal, ancillary and financial staff to assist in resolving overlayissues of multiple patient information in one medical record to resolve overl ay within 24 hours of discovery of overlay. 15%
- Participates in 24-ho ur on-call rotation. 15%
- Maintains enterprise master pat ient indexintegrity medical record number merges, un-merges and medical record numberassignments electronically. Perf orms name anddemographic changes with supporting documentation. 5%
- Works with management to compile and review reports ofduplicat e numbers, overlays, contact moves errors and identifies patternsrelated to user and type of error. 5%

Minimum Qualifications
Education
Associate's degree in a health information management required
Experience
2 years' experience in Health Information Management required Bachelor's Degree in HIT Preferred
Degrees, Licensures, Certifications
RHIT or eligible to sit. RHIT must be obtained within 6 months of hire.
